چکیده

Idiopathic pneumonia syndrome (IPS) is a significant noninfectious complication of hematopoietic stem cell transplantation (HSCT). We compared the incidences and outcomes of IPS among patients who underwent allogeneic HSCT after nonmyeloablative (n 183) compared with conventional (n 917) conditioning between December 1997 and December 2001. Patients given nonmyeloablative conditioning were older than those given conventional conditioning (median ages, 53 vs 41 years; P .001). The cumulative incidence of IPS was significantly lower at 120 days after nonmyeloablative conditioning than conventional conditioning (2.2% vs 8.4%; P .003). In addition, greater patient age (older than 40 years), diagnosis of acute leukemia or myelodysplastic syndrome, and severe acute graftversus-host disease were associated with significantly increased risks for IPS. Among older patients (older than 40 years) given conventional conditioning, highdose total body irradiation (TBI) was associated with an increased risk for IPS than were non-TBI–based regimens (16% vs 5.8%; P .001). IPS occurred early after transplantation, progressed rapidly, and was associated with a high mortality rate (75%) despite aggressive support. Initiation of mechanical ventilation and the presence of renal insufficiency at IPS onset were associated with increased risks for death after IPS. These findings support the concept that lung damage from the conditioning regimen plays a crucial role in the development of IPS after HSCT. (Blood. 2003;102:2777-2785)

CLINICAL OBSERVATIONS, INTERVENTIONS, AND THERAPEUTIC TRIALS Risks and outcomes of invasive fungal infections in recipients of allogeneic hematopoietic stem cell transplants after nonmyeloablative conditioning

The incidence of invasive mold infections has increased during the 1990s among patients undergoing allogeneic hematopoietic stem cell transplantation (HCT) after myeloablative conditioning. In this study, we determined risk factors for invasive mold infection and mold infectionrelated death among 163 patients undergoing allogeneic HCT with nonmyeloablative conditioning. The cumulative incidence...

CLINICAL OBSERVATIONS, INTERVENTIONS, AND THERAPEUTIC TRIALS Incidence and outcome of cytomegalovirus infections following nonmyeloablative compared with myeloablative allogeneic stem cell transplantation, a matched control study

Nonmyeloablative allogeneic hematopoietic stem cell transplantation (HSCT) is increasingly being explored as therapy in patients who are not eligible for conventional myeloablative HSCT. Whether these transplants are associated with reduced risk of transplantation-related infections is unknown. We analyzed the incidence of posttransplantation cytomegalovirus (CMV) infections in 56 consecutive m...

CLINICAL OBSERVATIONS, INTERVENTIONS, AND THERAPEUTIC TRIALS Nonmyeloablative allogeneic stem cell transplantation for the treatment of chronic myeloid leukemia in first chronic phase

Reduced-intensity or nonmyeloablative stem cell transplantation (NST) is designed to induce host-versus-graft tolerance by engraftment of donor stem cells. The rationale behind NST is to induce optimal graft-versus-leukemia (GVL) effects for elimination of all malignant cells by donor alloreactive immunocompetent cells as an alternative to standard highdose myeloablative chemoradiotherapy. NST ...

CLINICAL OBSERVATIONS, INTERVENTIONS, AND THERAPEUTIC TRIALS Engraftment and survival following reduced-intensity allogeneic peripheral blood hematopoietic cell transplantation is affected by CD8 T-cell dose

The influence of graft composition on clinical outcomes after reduced-intensity conditioning is not well-characterized. In this report we prospectively enumerated CD34 , CD3 , CD4 , and CD8 cell doses in granulocyte colony-stimulating factor– mobilized peripheral blood mononuclear cell (G-PBMC) allografts in 63 patients who received transplants following nonmyeloablative conditioning with total...

CLINICAL OBSERVATIONS, INTERVENTIONS, AND THERAPEUTIC TRIALS High incidence of cytomegalovirus infection after nonmyeloablative stem cell transplantation: potential role of Campath-1H in delaying immune reconstitution

Nonmyeloablative conditioning is increasingly used for transplantation in a wide range of diseases, but little is known about its impact on the incidence of infections and immune reconstitution. We examined the pattern and outcome of cytomegalovirus (CMV) infections monitored by polymerase chain reaction–based assays and treated preemptively in 101 patients following nonmyeloablative conditioni...
